Case Management

Through a collaborative approach our experienced Care Managers liaise with you, your family, your GP and other health care providers. This involves the process of assessment, planning, facilitation and advocacy for options and services to meet your personalised needs.

We take the time to get to you know you, and continue to have formal reviews. We do not have a ‘set and forget’ approach to any care plan. Our Care Managers are made up of a team of professional nurses who are actively involved and ensure the outworking of your care optimises your health and wellbeing.

Holistic quality care is at the heart of the Sue Mann way and our case management ensures your care plan meets your needs now, and as they change.

Living with Dementia

The journey of living with, or caring for someone with dementia is often challenging. Difficulties arise which may be worrying and saddening, particularly with the fear of not knowing what lies ahead.

In Australia it is becoming increasingly common where one in ten people over 65, and three in ten over 85 have dementia.

Sue Mann Nursing and Community Care has 30 years experience in supporting people living with dementia and we offer a strong understanding and unique insight into the personal journey of those living with dementia, or caring for someone who does.

We recognise each of our clients living with dementia as a respected and valued person and encourage positive involvement in decisions and changes that are made to their care. Our care team are trained and experienced in:

  • recognising the early signs
  • providing support for diagnosis
  • providing everyday support for those living with dementia
  • supporting and educating family and carers
  • managing dementia specific care plans
  • providing referrals for support groups and services
  • regularly communicating to the client’s GP

Our person-centred approach to dementia care focuses on getting to know our client’s personal and individual story and unique likes and dislikes. We encourage remaining active in everyday tasks and activities where practical, to help maintain a sense of identity and promote confidence and self-esteem.

Remaining at home, in familiar surrounds when living with dementia can be assuring and comforting. The SMNCC team can assist with supporting everyday tasks such as personal care, domestic assistance, nutrition and hydration, social support and nursing needs.

We also recognise the vital role carers play in looking after someone living with dementia – it can be stressful, exhausting and demanding. SMNCC can provide support to organise for you to have a break, reenergise and return to care for your loved one.

Wound Care

Under the direction and experience of our Registered Nurses (RN), Sue Mann Nursing and Community Care offer a holistic approach in treating clients with wounds.

Through on-going education and contemporary wound care products, our nurses ensure best practice principles are followed.

Clients are thoroughly assessed and regularly reviewed, and with progressive team communication (including your local GP), wounds are effectively healed through a Continuity of Care.

We have trusted relationships with local specialist wound care clinics and advisors if needed. Our nurses understand and are experienced with advising on appropriate nutrition associated with wound care and effective pain management control to ensure you remain comfortable in your own home.

Chronic disease management

Chronic disease can affect people at any stage of their life and is now the leading cause of death and disability in Australia . However, as we age, the chance of having a chronic disease increases, with those aged 85 years and over have at least one long-term condition.

Whether it be diabetes, arthritis, heart disease or dementia, the Sue Mann team can assist you to self-manage your chronic disease at home.

Our team work collaboratively with those living with a chronic disease, their families and carers to:

  • understand their condition (s) and treatment options
  • develop a personalised care plan
  • present and co-ordinate activities to promote health
  • assess and review symptoms and signs of the condition(s)

Our holistic approach to client care also allows us to integrate the management of chronic diseases with the overall care plans of our clients.

Palliative care at home

Bringing reassuring comfort and skilled nursing experience to the challenges surrounding palliative care is where Sue Mann’s caring team can help.

Palliative care is an approach that is about living for as long as possible in the most satisfying way you can within the limits of your illness.

Our Case Managers provide in-home and remote support (via phone) to palliative care clients, their families and carers as and when they need it. Our holistic palliative care approach focuses on:

  • Providing dignity to maintain quality of life
  • Controlling pain and relieving suffering
  • Liaising regularly with the client’s GP and specialist palliative care teams
  • Supporting family members
  • Integrating the physical, emotional, psychological and spiritual aspects of client care
  • Affirming the process of end of life

The Sue Mann nursing care team members support clients, their caregivers and families through the full journey of a life-limiting illness.
